How to Apply for A Program

This tutorial will show you how to complete an application for any of the offered Center for Community Partnerships Programs.
You must complete the entire application at one time. Your information will not be saved if you come back later.
Navigation
First, navigate to https://apps.chhs.colostate.edu/ccpparticipant/apply or click on the Apply navigation button.
Next, locate the program which you wish to apply for and click on the green Apply button.
Enter your CSU ID number if you have one, or continue without a CSU ID.
On the Applicant Information page, fill out the required information and as much other information as you can provide, then click Save and Continue at the bottom of the form.
You will then be directed to the specifics of the program you selected. Fill out the form and click Continue at the bottom of the form.
Consent
Some program applications require consent from you, the participant. If your program requires consent, read the page, then consent or decline and continue.
You will then be directed to the confirmation page.
