How to Complete the Video Consent Form

This document explains how to complete the video consent form for student teachers. This form only needs to be completed once.

Instructions

1. Login to your student teaching account (https://apps.chhs.colostate.edu/CEP)

2. If you have not yet filled out the consent form you will see a banner prompting you to fill it out. Click “complete this form” to proceed.

1. Read through the consent form, check “I Consent” and then click the “Consent” button.
