Documentation for the Campus Connections Youth Referrals process which uses MS Forms and Power Automate to capture application data, send custom emails, and append application data to a shared Excel spreadsheet.

In June 2021 we retired the outdated ASP.NET application and replaced it with an MS Form. There are several Office 365 tools utilized with the new set up and this guide illustrates how the different pieces work together. 

Office 365 Shared Account

All tools (MS Form, Power Automate, Outlook, MS Excel) used in the application process were set up under the Campus Connections shared Office 365 account chhs-hdfs_campus_connections@colostate.edu (CHHS IT - password is in KeePass). The following people have been given access to view the files created under the shared account and these users should be able to access by signing in with their own Office 365 credentials:
